How could I apply to a college, if I take a year off in between high school and freshman year in college?

I have maintained a 95 average throughout my high school career. However, I would like to take a year off from school and focus on other things due to family problems. I would like to know how a college would obtain my record and transcript if I decide to enroll a year after I graduate high school.
